We here at IACH appreciate your interest in becoming a member. Please print and fill out the application, attach a check in the amount for the membership desired made out to IACH and mail it to:
NOTES: Pro ration of fees are applicable from October 1st through June 30. Please call if applicable to you. A $15.00 charge may be required for returned checks. There shall be a sixty (60) day grace period for renewal of membership. After said grace period, RE-TESTING AND/OR INITIATION FEE may be required upon determination of the Certification Committee.
